The Town of Islip is seeking nominations for its 2014 celebration of Black History in February and is looking to honor individuals who have made significant contributions to the Islip community, and live or work in the town.

Applications can be mailed to the Department of Parks & Recreation, Attn: C. Charchalis, 50 Irish Lane, East Islip, NY 11730, faxed to (631) 224-5316, or e-mailed to jgabriel@townofislip-ny.gov. In addition, a short biography, resume and letters of recommendation must be attached to the nomination form.
Honorees will be selected based upon their accomplishments and contributions to the Town of Islip Community.
Nominations must be received by Tuesday, January 7, 2014
